Motor neglect (MN), which denotes an underutilization of the contralesional limbs not explained by sensorimotor deficits, and premotor neglect (PMN), a directional disorder of movements towards or in the contralateral hemispace, are functionally disabling but underdiagnosed syndromes which affect up to a third of stroke survivors. However, reliable clinical tests are still lacking as the differentiation from primary sensorimotor deficits and visual neglect is challenging.
Primary objective:
Validation of adapted and novel tests of upper limb function and gait can for the identification of MN and PMN and differentiation from primary sensorimotor deficits and visual neglect. This will improve diagnostic assessments of PMN and MN following stroke.
Secondary objective: Improve the understanding of PMN and MN related functional impairments, their prevalence and neural correlates.
The proposed study is a single center, exploratory, case-control study with a longitudinal follow-up of stroke patients.
The intention is to include 200 stroke patients (100 per lesion side) and 160 controls (patients with transient ischemic attack (TIA) and healthy volunteers). We aim to re-investigate at least 40 stroke patients with signs of either PMN, MN or visual neglect in a 3-month follow-up visit. Patients in the acute stage following ischemic stroke will be included in the research project. This is necessary to accurately investigate stroke related deficits.
