The present investigation shall generally help to assess the feasibility, functionality and effect of a new
digital health application (Carity-P) in the target patient group, coupled with a wearable monitoring
device that is designed to help patients after a cardiovascular event to monitor their physical and
psychosocial health, define and track their objectives, keep connected to their care team, and after
rehabilitation ensure to remain motivated in prolonging the structure given by the initial rehabilitation
program.
